EXAMPLES OF CHANGING MEDICATIONS

Work through the following case examples of how to change from one opioid to another. Feel free to use the equianalgesic table and calculator provided for you in the same page. The point of the examples is to get comfortable working with the table, not to memorize it!

Case1: C.T., a 66 year old homemaker with breast cancer

Case 2: J.S., a 67 year old artist with non-small cell carcinoma

Case 3: D.M., a 71 year old retired Chrysler worker with metastatic colon cancer
