Work through the following examples of how to change from one opioid to another. Feel free to use your equianalgesic table and calculator. The point of the examples is to get comfortable working with the table, not to memorize it!

J.S., a 67 year old artist with non-small cell carcinoma

15 mg morphine q 4 hours during the day and 30 mg at night

 10 mg morphine q 4 hours during the day and 20 mg at night

 5 mg morphine q 4 hours during the day and 10 mg at night

 20 mg morphine q 4 hours during the day and 40 mg at night

*Fentanyl should not be used in opioid naive patients. Use manufacturer's table for dosing after patient is established on another opioid.
